THE BEST FOOTBALL CLINIC IN THE VALLEY IS BACK AT BASD STADIUM.

The Five Star Heart Project fivestarheart.org and the Dawkins Family Foundation dawkinsfmilyfoundation.org invite football players entering grades 5th, 6th, 7th, and 8th in the Fall of 2025 to unite and participate in a FREE football clinic 5-Star Heart Style led by Five Star Heart's volunteer staff, the Navy Football Brotherhood, Lehigh Valley Health Network's Impact Athletes to include Penn State Football Captain and 2024 Allstate Wuerfel Trophy Recipient Nick Dawkins along with Mason Kuehner (Lafayette), Sean Wilson (Lafayette), Ethan Hosak (Lafayette), Jake Stalsitz (Lehigh), Sam Ayache (Lehigh) and Matt Machalik (Lehigh) in addition to a cast 25+ college football players from Penn State, Lehigh, Lafayette, East Stroudsburg, Moravian, and other institutions ready to coach and inspire the next generation of aspiring student athletes.

We will be raffling off NIKE apparel and signed memorabilia at the clinic to raise money for The Five Star Heart Project and The Dawkins Family Foundation. We will be accepting cash, Venmo and Cash App.

Those student athletes who showcase the most HEART throughout the clinic will be afforded with another experience to be announced.

*All youth organizations and middle school teams represented by a minimum of twenty (20) student athletes are eligible to compete in the team speed sled race. Student athletes from the winning middle school team must not be failing a class to earn their seat on the bus at the time of the trip scheduled for Saturday, November 22nd. Five Star Heart can accompany a maximum of forty-five (45) student athletes from the winning team on the trip.

I CONSENT TO SUCH USES AND HEREBY WAIVE ALL RIGHTS TO COMPENSATION.

PARENT AUTHORIZATION AND GENERAL RELEASE FROM LIABILITY

I AM REGISTERING FOR MYSELF OR ON BEHALF OF MY CHILD AND I AM AT LEAST 18 YEARS OLD, LEGALLY COMPETENT, AND I UNDERSTAND AND AGREE THAT THE TERMS BELOW ARE CONTRACTUAL. I UNDERSTAND THAT THERE ARE INHERENT RISKS IN THIS ACTIVITY, WHICH HAVE BEEN CONSIDERED AND WHICH THE PARTICIPANT ASSUMES. PARTICIPANT HAS MEDICAL INSURANCE. I CONSENT TO EMERGENCY TREATMENT FOR MY CHILD, IF IN THE JUDGEMENT OF THE FIVE STAR HEART PROJECT, IT IS REQUIRED.
